Special Instructions:
Only licensed Electrician/Plumber may contact the permit office to receive a permit/approval. The customer is never to call the permit office

As of June 1992, The State of Louisiana passed a law which states, “No permit is required to connect or re-connect a traffic signal light located on state property” (i.e. highways, buildings, etc.)
Effective September 14, 1995.
Whenever a permit or approval is required on a building with an address of upper rear or lower rear etc.—that address must be changed to read Apartment. A, B, C, D, or Apt .1, 2, 3. The owner will be required to send a letter to the building department of the city requesting the address be changed. Upper or lower rear are no longer recognized as valid addresses when filing for a permit. This same rule applies for vacancy permits. Also, meter bases should be marked correctly.
Do not tell the electrician to re-file for the permit using upper or lower rear.
State Requirement
Before clearance can be given on a highway or waterway for moving a house, etc. The State now requires a permit before the work can be done.
Electrical permit or inspection for one or more of the following:
Vacant longer than 6 months for residential & commercial
Permit expires after 6 months for residential & commercial
New construction, temporary meter.
New construction, permanent meter
Existing structure-if Entergy has to run a service line a permit is required
Fire
Electrical code violation
Rewiring/increase in service
Meter pan/base or loop change/relocation
Work inside meter pan or repairs made to mast or weatherhead
To have service restored after service has been disconnected for customer to do electrical repairs
Connect or reconnect a traffic signal light (See Louisiana State Property Exemption Law in special instructions above).
Gas approval or inspection for one or more of the following:
Vacant longer than 6 months for residential, commercial
Approval expires after 6 months for residential, commercial
New construction or installation
Upgrade of gas service
Gas code violation
Fire
Meter or gas/fuel line relocation
Disconnected gas/fuel line
All gas leaks
